Job Description

This position provides personal care to residents to ensure the highest degree of quality care at all times within a safe environment. Assists with activities of daily living, offers emotional and social support, and attends to residents' overall physical well-being.


Functions and responsibilities


  • Assist with implementation of each individual's Service Plan, therapeutic physical/psychological care, and increasing independent vocational and leisure skills.


  • Assist with the implementation of restoration plans.


  • Maintain documentation of behavioral symptoms and other data collection requirements in accordance with the Service Plan.


  • Assist and promote independence with care (including personal hygiene), ambulation, meal preparation, personal laundry, and housekeeping.


  • Serve as an integral member of the team by providing appropriate interventions during medical, behavioral, and/or facility emergencies.


  • Provide personal hygiene including bathing (bath, shower, bed), oral care, skin and nail care, and hand washing.


  • Complete grooming of residents including hair care, shaving, dressing and weighing. Encourages residents to take care of themselves independently.


  • Assist with toileting including bowel and bladder retraining and perineal care.


  • Perform and assist with mobility, transfer, ambulation, positioning and range of motion according to the plan of care.


  • Accurately document all care provided, including dietary intake and excretion, temperature, weight, using approved abbreviations and descriptive terms.


  • Serve meals to residents; motivates and helps with food and fluid intake.


  • Inform supervisor of written and verbal assignments, including daily use of care plans for assigned residents prior to initiating care

Requirements


  • Successfully complete the Certified Nursing Assistant (CNA) training course.

  • Wisconsin Certification as a Certified Nursing Assistant and on state registration.

  • Successfully pass the caregiver and criminal background check.
